These NFMC forms are required of each TFMC Federation Festival event. Submit them each September or no later than the deadline communicated by the TFMC State Festival Chair.

NFMC Certificate of Liability
Festival Event Liability Coverage
Each festival event should have adequate liability insurance. District festival chairs should complete and submit this form whether the festival venue requires a Certificate of Liability or not. Send to TFMC; TFMC requires the data.

If you do not have a festival date set at this time, please estimate the date and location. Let the TFMC State Festival Chair know as soon as final details become available.
If there are multiple festival locations or dates for a festival district, please submit this form for each location and/or date.
A late fee will be incurred when requesting festival coverage after the published deadline.
NFMC Child Protection Certification - Hard Copy
This certification is required for each TFMC Festival Event
Information requested on this form must be provided and the form must be signed before any festival insurance requests will be processed.

NFMC "Superior" Certificates and Theory Tests
Provide this information each year by September 15th.
Confirm the number of "Superior" and "Consecutive Superior" Certificates for your TFMC Festival District. Indicate whether test materials are needed for your festival's Theory Event.
Certificates are provided by NFMC at no charge.
The TFMC State Festival Chair is responsible for ordering all certificates and theory test materials from NFMC. Hard-copy packets should be requested via email to the TFMC State Festival Chair. TFMC provides theory test sets in pdf format at no charge. The hard-copy sets cost $2.50 (plus shipping).
